40th Annual Forest Park STAR Invitational

TEAM CHECK-IN

All teams are required to check-in prior to the tournament.  There will be no registration at the fields.  Team check-in will be held:

WHAT TO BRING TO CHECK-IN

  • TEAM ROSTER (2 copies, kept by the tournament)

  • PLAYER CARDS

  • COACH CARDS

  • GUEST PLAYER ROSTER and player cards

    • Ohio South teams go to OSYSA.com., click on Forms and Resources, then E-travel system.

    • Kentucky teams, go to http://www.kysoccer.net

    • Others, go to your state association

    • Don't forget medical release for guest players!

  • MEDICAL RELEASE FORMS

    • A form is required of each player signed by the parent or guardian.  (Notarization is not required).

    • You may use forms from your own club or State Association.

    • Player membership forms are not acceptable

    • If you do not already have a release form, download one here.

  • TRAVEL PERMITS

    • Teams from outside Ohio South are required to provide an approved travel permit from their respective State Association.

    • Kentucky teams, go to http://www.kysoccer.net

    • Kentucky and Indiana teams in the Cincinnati area:  This includes you!

 

NOTES ON GUEST PLAYER PERMITS

  • Maximum roster size may not be exceeded

    • 12 for 6v6; 14 for 8v8; 18 for 11v11

  • Number of guest players permitted

    • 3 for 6v6; 3 for 8v8; 4 for 11v11

  • "Trapped 8th-graders" are permitted.  However, you cannot have an over-age player as tournament guest.

TOURNAMENT-ONLY GUEST PLAYERS

(uncarded players)

A registrar from Ohio South will be on-site to process previously uncarded players.  These players will receive a player pass that is good for the duration of the tournament.  The following items are required:
